Your details, and what I do with them.
I visit people’s homes and I’m often told about their health, so I keep this short and plain. If anything here is unclear, ask me.
Who is responsible
Anaya Mehta, PT, trading as Signature Physio, Pune, decides how your details are used and is responsible for them. You can reach me at hello@signaturephysio.in or +91 00000 00000.
What I collect when you book a session
- Your name and mobile number, and whether the visit is for you or someone else.
- For sessions at home (physiotherapy or private yoga): the address, PIN code, an optional landmark and Google Maps link.
- The main reason for the visit, which days and times suit you, and anything you choose to add in the notes, which may include health information.
- How you heard about the practice, and the referring doctor or hospital if you name one.
Before or after the first visit you may also send me discharge summaries, prescriptions or scans on WhatsApp, and pay a refundable amount by UPI.
Why, and on what basis
Only to arrange and give your care: to call you back, plan the visit, find your home, and keep notes on your treatment. You give your consent by ticking the box on the request form. I don’t use your details for marketing and I never sell them.
Who else handles them
A few services are needed to run the form. Each receives only what it needs:
- Text message or WhatsApp provider: your mobile number, to send the 6-digit code.
- Cloudflare Turnstile: technical details of your browser, to check the form isn’t being filled by a bot.
- Resend, an email delivery service: your whole request, including any notes, so it reaches my inbox. It passes through Resend’s servers on the way.
- Website hosting: the request passes through the host’s servers. Its logs keep only a summary (the type of visit, reason, PIN code and a masked phone number), never your name, address or notes.
I don’t share your health information with anyone else without asking you first, unless the law requires it or someone’s life is at risk.
How long I keep them
Requests that don’t lead to a visit are deleted within 90 days. Treatment records are kept for as long as the law requires for clinical records, then deleted.
Your rights
You can ask me, at any time, to:
- tell you what details I hold about you and who they’ve been shared with;
- correct or complete them;
- delete them, where I’m not required to keep them;
- stop using them, by withdrawing your consent. This doesn’t affect care already given.
You can also name someone to act for you if you are unable to. Write to hello@signaturephysio.in; I’ll reply within 30 days.
Children and people you book for
If you book for a child under 18, you must be their parent or guardian. If you book for a parent or relative, please make sure they’re happy for you to share their details.
Complaints
If you’re unhappy with how I’ve handled your details, tell me first at hello@signaturephysio.in. If I can’t put it right, you can complain to the Data Protection Board of India.
